An immunization entry may use a brand name, an abbreviation or the diseases covered by a combined vaccine. Those distinctions should remain visible in translation. A trade name is not replaced with an assumed equivalent product.

Dose dates and handwritten entries

Several doses can appear in the same row or on different pages. The translation keeps each date attached to the corresponding vaccine and dose. If handwriting leaves a date ambiguous, provide a clearer image or an updated record from the issuer.

Lot numbers, facilities and signatures

Batch or lot numbers, clinic names, stamps and professional signatures document the recorded administration. Include readable entries in the margins and on the reverse. Blank spaces are not filled with inferred doses or dates.

Translation versus a medical assessment

A translation communicates what the vaccination record says. It does not determine whether immunizations are current, decide which additional doses are needed or replace an examination by an authorized medical professional. Ask the receiving institution which records it needs.
