A discharge summary, consultation note and laboratory report describe different encounters. Keep the provider, patient and encounter dates identifiable. The translation follows the type and scope of the record supplied.

Medication names, doses and units

Drug names, dose amounts, routes and frequencies require separate checks. A brand is not replaced with a presumed substitute medication. The translator does not recommend treatment or change a prescribed quantity.

Laboratory tables and reference ranges

Results must stay connected to the correct test, unit and reference interval. A reference range belongs to the issuing laboratory's report. Translation does not interpret a result as a new diagnosis.

Handwriting and sensitive attachments

Clinical abbreviations and handwritten notes can be difficult to read. Supply clear images and connected pages for the agreed scope. Missing or uncertain text is not completed by medical inference.
